I-9 Form

The Employment Eligibility Verification Form I-9 is a U.S. Citizenship and Immigration form. It is used by an employer to verify an employee's identity and to establish that the worker is eligible to accept employment in the United States.

 

 

W-4 Form

The Form W-4 is used by employers to determine the correct amount of tax witholding to deduct from employees' wages.

 

IT2104 Form

Form IT2104 is used by employers to determine the correct amount of tax witholding in NYS.

 

NYS Disability Form

Employers use this form when filing a claim for Disability.  Forms can also be obtained by your carrier.

 

C-2 Form

The form C-2 is used by employers to report illness or injury caused by work-related incidents. 

 

C-3 Packet

The C-3 packet is given to the employee when he/she is injured or becomes ill due to a work-related incident.

 

W-9 Form

Form W-9 is the IRS form used by a company to request your taxpayer identification number. You may get a blank Form W-9 to fill out if you or your business are hired to provide services to another company. Most often, Form W-9 is sent to independent contractors, consultants, and other self-employed workers.
